Katherine Plunkett, LMSW, MPH, CHES, PCMH CCE
Biography
In her role as Senior Manager of Clinical Quality Improvement at the South Carolina Primary Health Care Association, Katherine Plunkett provides technical assistance and training to community health centers around the topics of practice transformation, oral and behavioral health integration into primary care, and patient-centered medical home initiatives. Mrs. Plunkett serves as a resource regarding evidence-based standards of care and provides guidance to community health centers on improving health outcomes for patients.
Katherine Plunkett is a graduate of the University of South Carolina with a Masters Degree in Social Work and a Masters Degree in Public Health. Additionally, Mrs. Plunkett is a licensed social worker in the state of South Carolina (LMSW); a certified health education specialist (CHES); and a certified content expert in the patient-center medical home recognition process (PCMH CCE)
